New Student Enrollment Application

Kainos is required to have on file an official document showing your student's full legal name and date of birth. A copy of your student's birth certificate must be provided to Kainos before your student may attend classes. You may upload a copy here, or you may email a copy to Kainos at a later date.
Kainos offers two options for students: a full-time comprehensive academic program, as well as a hybrid program for homeschool families.

Full-time Enrollment
  • Comprehensive academic program, including all core subjects as well as project-based learning.
  • Kainos is the school/teacher of record.
  • Available for students in grades 1 - 12.
  • Monday - Thursday 8:30am - 2:30pm and Friday 8:30am - noon.
Hybrid Program
  • Instruction in writing, literature, science (labs), history & art, as well as some project-based learning. 
  • Homeschool (parent) is the school/teacher of record (Kainos provides instruction for subjects identified; the parent ensures student completes all work, grades all assignments and assessments, and keeps all records).
  • Available for students in grades 5 - 12 only.
  • Monday and Tuesday 8:30am - 2:30pm.
Student Academic History
School records will need to be provided to Kainos prior to your student's first day of attendance. Records should be sent to jill@kainosmicroschool.com. Kainos Administration can request these records directly; however, some schools will require the parent/guardian to grant permission for the release of records. 

** Homeschool families will need to provide a full detailed overview of the student's educational experience, including the school year, curriculum used, and the parent's assessment of mastery (i.e. letter grade, pass/fail, "completed," etc.), for each year the student was homeschooled. This document should be sent via email to jill@kainosmicroschool.com. 
Student Learning Interests and Styles

The following questions will help provide us with a better picture of your student. We encourage you to ask them to help you answer these questions.
How does your student learn best? (select all that apply)
Has your student ever had an IEP/ISP/504 Plan, or have they been diagnosed with a learning disability (including the need for speech therapy)?
If your student has an IEP/ISP/504 Plan, will they continue to be supervised by a public school Special Education Liaison?
Students who have an IEP/ISP/504 Plan qualify for the Indiana ESA program. Qualification for the program does not guarantee the student will be funded.
